  • Gifted Education Referral Form (PDF)

    Parents, guardians, students, peers, self, teachers, and counselors all have the ability to refer a student to be assessed for gifted education services. Either complete the referral form through the link (PWCS Gifted Education Program Referral Form) or download the form and return it to the gifted office in room 2135.

    Once the referral is received by the gifted office, the resource teachers will reach out to the student's parents to obtain permission to start the identification process.

    Specific information on the referral process is also available through the Gifted Education Program webpage accessible through the PWCS homepage. Forms for identification are translated into multiple languages.

  • Permission for Services Form (PDF)

    All students in the gifted program are required to have a permission for services grades 9-12 form on file in order to receive direct services from the gifted resource teachers. This form only needs to be signed and submitted once during the student's time in high school.

    Selecting the section that says "I do give permission" enables services to be received, while selecting "I do not give permission" exits the student from the program.
